Mandatory Self Assessment

At Petit Pearson the safety of our children, educators, and their families is our top priority.

It is mandatory to fill out the Covid-19 screening form below, for each child, every day including the date. In addition to this your child may still be screened in person and have their temperature taken. Anyone exhibiting symptoms of COVID-19 (Coronavirus) is expected to stay home and self isolate until they are deemed safe by a medical professional.

The effects of COVID-19 on children are only now just being discovered so please make sure to take the proper precautions such as washing hands and bringing masks/gloves for everyone’s safety and comfort.

Results of Screening Questions:
If you answered “YES” to any of the symptoms included under question 1:
• Your child should stay home to isolate immediately.
• Contact your child’s health care provider for further advice or assessment

• Get tested with a PCR test

If the you answered “YES” to questions 2, 3, 4 or 5
• Stay home + follow Toronto Public Health advice

    *Children/students who have an existing health condition identified by a health care provider that gives them the
    symptoms should not answer “Yes,” unless the symptom is new, different or getting worse.

    *If the child/student is 12+ years old and has a mild headache, tiredness, sore muscles or joints within 48 hours after
    getting a COVID-19 vaccine, they should select “No” and wear a fitted mask when at child care/day camp/school. If
    symptoms last longer than 48 hours or worsen select “Yes.”

    If “YES” to any symptoms: Stay home & self-isolate + get tested or contact a healthcare provider. Your household including siblings must also
    self-isolate, regardless of vaccination status

    • If the child/student has a health condition diagnosed by a health care provider that gives them the symptom,
    select “No”. If the symptom is new, different or getting worse, select “Yes”.
    •If there is mild tiredness, sore muscles or joints within 48 hours after a COVID-19 vaccine, select “No”.
    If longer than 48 hours, select “Yes.”
    • Anyone who is sick or has any symptoms of illness, should stay home and seek assessment from their health care
    provider if needed.
    *If child/student has one symptom from Part B stay home until symptoms improve for at least 24 hours or 48 hours if nausea/vomiting/diarrhea.



    If “YES” to questions 2, 3, 4 or 5: Stay home + follow Toronto Public Health advice

    *Use 5 days: If the person is fully vaccinated or 11 years or younger. Use 10 days: If they are 12 years or older and not fully vaccinated; or immune compromised.
    **Fully vaccinated means 14 days or more after a second dose of a COVID-19 vaccine series, or as defined by the Ontario Ministry of Health.
    ***Confirmed COVID-19 infection within 90 days means: if tested positive within 90 days on a Rapid Antigen, or a PCR test AND completed self-isolation.
    Then, do not need to self- isolate if someone in the home has symptoms.
